LASIK (laser-assisted in situ keratomileusis) is an outpatient surgical procedure used to treat myopia (nearsightedness), hyperopia (farsightedness) and astigmatism.


Modern LASIK surgery is now bladeless using a femtosecond laser to create a corneal flap. The surgeon folds the flap back, and then removes some corneal tissue using an excimer laser. The flap is then laid back in place. When the cornea is reshaped, it focuses light directly onto the retina, providing clearer vision.


The surgery takes about 15 minutes to perform for each eye. LASIK is performed using anaesthetic eye drops so there is no pain during the procedure. However, you may experience some eye discomfort and tearing during the first eight hours after the surgery. Common side effects are dry eyes, light sensitivity, glare and halos during the first few weeks, but most of these usually subside over time.

The LASIK Procedure – How is it performed?


At the start of surgery, anaesthetic eye drops are applied. Once the eye is numbed, an eyelid speculum is inserted to prevent the eyelids from blinking. Next, a thin flap is created in the outer layer of the cornea using a femtosecond laser. A little pressure or tightness may be felt at this stage.


Next, the hinged corneal flap is lifted, exposing the treatment area. You will be prompted to look and focus on an overhead target light while the excimer laser is applied to the treatment area.


Finally, the flap is repositioned upon completion and allowed to adhere, stitch-free. The eyelid speculum is removed at this stage. The whole treatment lasts approximately 15 minutes for each eye.


Once the anaesthetic eye drops wear off, the eye will feel quite gritty and irritable for a few hours. Frequent application of lubricating eye drops is useful. It is important that you do not rub your eyes in the first two weeks as the flap could be dislodged. Vision is misty during the first day and should clear by the next day. If there is severe pain with reduced vision, the flap may have been displaced and you should return to the eye centre immediately.

ICL (Implantable Contact Lens) is similar to contact lens, but instead of having the lens on the cornea (front part of the eye), the surgeon will implant the lens permanently behind the cornea in front of the natural lens.

Why does the intraocular pressure increase ?


Human eye has a fluid called the aqueous humor, this fluid drains from very fine canals right at the corner where the iris and the cornea meets. This system is very sensitive and any problem affecting this drainage will result in an increase in IOP and causes pressure on the nerve fibers of the optic nerve. Consequently, there will be damage to these sensitive fibers and permanent vision loss.


How Is It Diagnosed?


Your eye doctor will test your vision and examine your eyes. He’ll check your optic nerve, may take photographs of the nerve to help him track your disease over time.


He’ll do a test called tonometry to check your eye pressure. He’ll also do a visual field test, if necessary, to figure out if you’ve lost your side, or peripheral, vision.


An OCT test is a very useful test where the doctor can take a precise photo of your optic nerve fibers to see the affected fibers and to plan your treatment accordingly.


All Glaucoma tests are painless and take very minimal time.

Diabetic retinopathy is one of the diabetes complications, in which a damage of the fine blood vessels of the light-sensitive tissue at the back of the eye occurs. How is LASIK surgery performed?


First, your LASIK surgeon will create a very thin, superficial flap in your cornea with a small surgical tool called a microkeratome or with a femtosecond laser.


LASIK eye surgery: step-by-step


The surgeon then folds back the hinged flap to access the underlying cornea (called the stroma) and removes some corneal tissue using an excimer laser.


Excimer lasers create a cool ultraviolet light beam to remove (“ablate”) microscopic amounts of tissue from the cornea to reshape it so light entering the eye focuses more accurately on the retina for improved vision.


For nearsighted people, the goal is to flatten the cornea; with farsighted people, a steeper cornea is desired. Excimer lasers also can correct astigmatism by smoothing an irregular cornea into a more normal shape.


After the laser ablation reshapes the cornea, the flap is then laid back in place, covering the area where the corneal tissue was removed. The flap seals to the underlying cornea during the healing period following surgery.


LASIK laser eye surgery requires only topical anesthetic drops, and no bandages or stitches are required.


Before LASIK surgery


Your eye doctor will perform a thorough eye exam to ensure your eyes are healthy enough for the procedure. He or she will evaluate: the shape and thickness of your cornea; pupil size; refractive errors (myopia, hyperopia and astigmatism); as well as any other eye conditions.


The tear film on the surface of your eyes also will be evaluated, and a precautionary treatment may be recommended to reduce your risk of developing dry eyes after LASIK.


Usually, an automated instrument called a corneal topographer is used to measure the curvature of the front surface of your eye and create a “map” of your cornea.


With wavefront technology associated with custom LASIK, you also are likely to undergo a wavefront analysis that sends light waves through the eye to provide an even more precise map of aberrations affecting your vision.


Your eye doctor also will ask you about your general health history and any medications you are taking to determine if you are a suitable candidate for LASIK.


You should stop wearing contact lenses for a period of time advised by your doctor (typically around two weeks) before your eye exam and before the LASIK procedure. This is because contact lens wear can temporarily alter the natural shape of your cornea.


What to expect during LASIK


Before your LASIK surgery begins, numbing eye drops are applied to your eye to prevent any discomfort during the procedure. Your doctor may also give you some medication to help you relax.


Your eye will be positioned under the laser, and an instrument called a lid speculum is used to keep your eyelids wide open.


The surgeon uses an ink marker to mark the cornea before creating the flap. A suction ring is applied to the front of your eye to prevent eye movements or loss of contact that could affect flap quality.


After the corneal flap is created, the surgeon then uses a computer to adjust the excimer laser for your particular prescription.


You will be asked to look at a target light for a short time while your surgeon watches your eye through a microscope as the laser sends pulses of light to your cornea.


The laser light pulses painlessly reshape the cornea, although you may feel some pressure on your eye. You’ll also hear a steady clicking sound while the laser is operating.


LASIK is performed on each eye separately, with each procedure taking only about five minutes.


Immediately after LASIK surgery


Upon completion of your LASIK procedure, your surgeon will have you rest for a bit. You may feel a temporary burning or itching sensation immediately following the procedure.


After a brief post-operative exam, someone can drive you home. (You cannot drive after LASIK until your eye doctor sees you the following day and confirms your uncorrected vision meets the legal standard for driving.)


You should expect some blurry vision and haziness immediately after surgery; however, clarity should improve by the very next morning.


Your eyesight should stabilize and continue to improve within a few days, although in rare cases it may take several weeks or longer. For most people, vision improves immediately.


You may be able to go to work the next day, but some doctors advise at least one day of rest instead.


Also, it is usually recommended that you refrain from any strenuous exercise for at least a week, since this can traumatize the eye and affect healing.


Generally, you will return to see your eye doctor or your LASIK surgeon the day after surgery.


At this initial check-up, your visual acuity will be measured to make sure it is safe for you to drive without glasses or contact lenses. In most states, this requires visual acuity of 20/40 or better.


As with any other surgery, it’s very important for you to follow your doctor’s instructions and take any medication prescribed.


Also, avoid rubbing your eyes, as there’s a small chance you could dislodge the corneal flap if you rub your eyes vigorously before the flap has securely reattached to the underlying cornea stroma.


Long-term results


Laser eye surgery offers numerous benefits and can dramatically improve your quality of life.


Most people achieve 20/20 vision or better after the surgery, but LASIK results do vary. Some people may achieve only 20/40 vision or less.


You may still need to wear glasses or contact lenses following laser vision correction, though your prescription level typically will be much lower than before.


If you have mild residual refractive error after LASIK and you want sharper vision for certain activities like driving at night, prescription lenses with anti-reflective coating often can be helpful.


Also, if you are sensitive to sunlight after LASIK, eyeglasses with photochromic lenses can reduce photophobia for clearer, more comfortable vision.


While LASIK has an excellent safety profile, LASIK complications can occur. These include infection and night glare (from starbursts or halos appearing around lights).


A small percentage of people will need a LASIK enhancement, or “touch up” procedure, a few months after the primary LASIK surgery to achieve acceptable visual acuity.


You also may still need reading glasses once you reach your 40s, due to a normal age-related loss of near vision called presbyopia.


While LASIK surgery has a high success rate, it is important that you discuss with your eye doctor or surgeon any concerns you may have before you consent to having the surgery.

LASIK in it’s most advanced form as performed at Sharif Eye  Center Laser utilises two separate lasers, a Femtosecond laser creates a hinged flap that is elevated and the base underneath the flap is remodelled with an excimer laser. The flap creation allows for next day vision recovery. The excimer laser that performs the reshaping element allows us to utilise the most advanced treatment algorithm, Contura Vision, that simultaneously perfects the corneal shape whilst correcting the glasses prescription. This has allowed for a new paradigm of visual quality outcomes, representing the first time one can achieve vision 2 lines better than the vision achievable with glasses.


SMILE is an attempt to use just a Femtosecond laser to perform the entire correction as opposed to just the flap creation. The femtosecond laser cuts a lenticle of tissue from the substance of the cornea that is then extracted through an access incision.


Both correct Myopia. SMILE was originally conceived to create less dry eye and potentially have a reduced risk of an unstable cornea (ectasia.) As time has progressed and a more accurate evaluation of the efficacy of SMILE has emerged, it appears these advantages have not been correct. Studies have shown an incidence of both dry eye and ectasia that is statistically similar to LASIK.


Considering SMILE does not have advantages over LASIK, what about its disadvantages?

Firstly it is inherently less accurate than Femtosecond LASIK with Contura Vision.


Approximately 98% of patients achieve vision equal to or better than 20/20 vision after a single treatment at Sharif Eye  Center with Contura Vision LASIK


The ReLEx SMILE only achieves 20/20 vision in about 60-85% of patients in recent studies. The inherent inaccuracy of ReLex SMILE stems from the fact the smallest amount of tissue removed with a femtosecond laser is to 4 microns compared to the 0.25 microns achievable with an Excimer laser. This equates to at least 16 times finer accuracy for an Excimer Laser.


This inherent lack of accuracy with SMILE also explains the increased frequency of requiring enhancement, at up to 40% in studies vs less than 2% at Sharif Eye  Center Unlike the next day recovery with minimal discomfort that can be achieved with Femtosecond LASIK, the enhancement is performed with PRK or surface based laser that is painful; for over 2 days and can take 6-8 weeks to recover.


Even when SMILE is effective, it generally takes 3 weeks to settle as opposed to next day recovery encountered with LASIK.


SMILE has a significantly reduced treatment range. It can only effectively correct up to 0.5 diopters of astigmatism compared to 6 diopters with LASIK. It cannot correct hyperopia or mixed astigmatism. It is incapable of the sophisticated surface perfecting ability of Contura Vision and as a result, can not provide this level of visual quality.
